This notice describes the driver application form on this website.

What the form collects

Your name, email address, optional phone number, answers about driving experience and training, availability, referral information, any experience summary you provide, and your confirmation. The site also records the submission time and an application reference.

How it is used and stored

BD Wheelchair Transport LLC, operating as BD Mobility, uses this information to review your interest in driver opportunities and to contact you about your application. Submissions are stored in the website’s database. The private application inbox is restricted to the site owner’s authorized account. When a new application is saved, Google Workspace sends the team a brief alert containing the applicant’s name and application reference; the remaining answers stay in the private inbox. Applicants do not receive an automatic email copy.

What not to include

Do not submit Social Security numbers, driver’s license numbers, financial account information, medical information, or copies of identity documents through this form.

Contact info@bdmobility.com if you have questions, need to correct an application, or want to request removal of your information. Include your application reference if you have it.
